PAGPAPATALA/PAGPAPALISTA NG PANGALAN

PAGPAPASURI NG MGA DOKUMENTO:

3.1 BIYAHE PAPUNTANG IBANG BAYAN

  • Permiso mula sa Punong Barangay
  • Health pass ng mga bibiyahe (ito ay may bisa lamang sa loob ng 5 araw)
  • Plate no. ng sasakyan

3.2  BIYAHE PARA SA MAMIMILI AT/O MAGDEDELIVER NG MGA PANINDA

  • Xerox Copy ng Mayor’s Business Permit
  • Xerox Copy ng DTI Registration
  • Official Receipt of Quarterly/Annual Tax Payment
  • Plate No. ng Sasakyan
  • Health Pass ng mga Bibiyahe

3.3 BIYAHE PABALIK NG TRABAHO

  • Employee’s Order to Report Back to Work
  • Certificate of Employment
  • Company ID
  • Certification that the Workplace Adheres to health standards and/or any document showing that the employee is safe to report back to work
  • Permit from the Punong Barangay Allowing his/her resident to work outside the locality
  • Health Certificate/Pass

3.4 BIYAHE PABALIK SA SARILING TAHANAN O IBANG PROBINSYA

  • Copy of List of Locally Stranded Individual (LSI) mula sa Barangay
  • LSI Registration form (Annex A)
  • Health Clearance
  • Notice of Availability of Travel from the LCE
  • PNP Clearance

KAPAG NATAPOS ANG BILANG 1-3, MAAARI NANG KUMUHA AT/O MAISYUHAN NG TRAVEL PERMIT/PASS

PAALALA:

  • Pumila lamang sa araw na nakatalaga sa Home Quarantine Pass
  • Kaalinsabay ng pahintulot ay awtomatikong Home Quaratine (14 Days) ang mga magsisipagbiyahe
  • Maaaring magbiyahe ang may edad 21 hanggang 59 ayon sa kahalagahan ng gagawin
  • Kapag babalik sa trabaho sa labas ng bayan papayuhang manatili sa lugar na pagtatrabahuan.
  • Limitado lamang ang maaaing mabigyan ng travel permit sa bawat araw.
  • Maaari lamang magpunta sa Lucena City tuwing miyerkules at biyernes , at sa Gumaca naman ay tuwing miyerkules, maliban emergency case/s.
