Requirements:
– Accomplished Application Form w/ recent 2×2 picture
– Signed Letter of Intent of the Student
– Barangay Certificate of Indigency
– Copy of Grades
– For incoming Freshmen, High School Report Card
– For applicants w/ earned units in college, summary of grades (2nd Sem 2020-2021)
– Copy of Registration/Enrollment Form-1st Sem 2021-2022
Isumite po ang mga kinakailangang dokumento sa Tanggapan ng Punong Bayan (PS Yeye).
